MEMBERSHIP APPLICATION

Individuals who are Current Members should renew their membership by logging into their member account through the "Log In" box in the upper left corner of any page on OPHA's website. 

Individuals can join OPHA through the membership form below. Please select the appropriate membership level (Regular, Regular 2-year, Student, Community Health Worker, or Retired).

Individuals whose groups or organizations are existing Organizational Members (see bottom of this page) should claim their membership for free through their organization with the form below. Please select the "Organizational - $60 (USD)" level and input the discount code provided to you by your group. Unsure if your organization is an existing member? View our Organizational Members at the bottom of the page.

Organizations interested in applying for Organizational Membership cannot apply using the form below. Please view and complete our Organizational Membership Form to view organizational membership rates and submit your application. Once accepted, individuals covered by your group membership will return to this page to claim their membership for free with your group's discount code through the "Organizational - $60 (USD)" level.
