Individuals

    • Current members should renew their membership by logging into their member account through the "Log In" box in the upper left corner of any page on OPHA's website. The email associated with your account is your username. Need to reset your password? You may do so here.
    • New members can join OPHA through the membership form below. Please select the appropriate membership level (Regular, Regular 2-year, or Discount).
    • If your group or organization has an Organization Membership with OPHA, you should claim their membership for free through their organization with the form below. Please select the "Organization - $65 (USD)" level and input the discount code listed by your organization. You will not be charged if you use your correct discount code!

Organizations interested in applying for Organization Membership cannot apply using the form below. Please complete our Organization Membership Application and email it to admin@okpublichealth.org. Do not submit payment with application. OPHA's Board of Directors reviews and approves applications at the monthly Board meeting, each second Friday of the month. Once accepted, you will receive an invoice for payment and individuals covered by your group membership will return to this page to claim their membership for free with your group's discount code through the "Organization - $65 (USD)" level.
