Registration

(depends on selected options)

Base fee:
  • Attendees that will only attend the Symposium.
  • One employee per each company, business, organization is allowed complimentary two-day registration. Additional staff who wish to work the exhibit booth must follow the standard registration process and pay the member/non-member fee.
  • OPHA members who register to attend the full conference May 8-9
  • OPHA members who registration attend the conference on May 8 OR May 9 ONLY.
  • Member student/retiree who register to attend the full conference May 8-9
  • Students/retiree OPHA Members who register to attend the conference on May 8 OR May 9 ONLY
  • Non-members who register to attend the full conference May 8-9
  • Non members who register to attend the conference on May 8 OR May 9 ONLY.
  • Non-member students/retiree who registers for the full conference May 8-9
  • Non member student/retiree who registers to attend the conference on May 8 OR May 9 ONLY.
  • ODMHSAS Central Office employees who are members of OPHA may select this option for payment.
  • Employees who work for the OCCHD and whose membership and conference registration is invoiced to OCCHD.
  • OSDH employees who are current OPHA regular members may charge their member-registration fee to the OSDH Purchase order. OSDH will not pay the non-member registration fee. A cap has been set by OSDH. Once this cap is reached, no additional registrations will be charged to OSDH.
  • You must be a member and a confirmed poster presenter in order to use this registration type.
  • Conference sponsors may select this type to register their allotted conference attendees.
  • Employees of TCCHD whose membership and registration fee is invoiced to TCCHD.

By submitting your registration for the conference, you agree to the terms of registration and limitation of liability.

Cancellations must be sent in writing to the OPHA office.  Cancellations received by April 30 will receive a 100% refund of their registration fee.  Cancellations received after April 30 will receive a 50% refund of their registration fee.



Hotel Reservations: Conference attendees are responsible for making their own hotel reservations

Continuing Education Credits:

This
program qualifies for Physician CME credit in the category of “Elective Enrichment CME Credit”. Physicians can report their actual hours of attendance to their professional organizations.

The University of Oklahoma College of Nursing is an approved provider of continueing nursing education by Colorado Nurses Associaion, an accredited approver by the American Nurses Credentialing Centers' Commission on Accreditation. UOC# 0616 - Approval active May 31, 2013 - June 30, 2016.
The University of Oklahoma College of Nursing is a co-provider of the 75th Anniversary Oklahoma Public Health Association Conference.

The Institute for Mental Health and Substance Abuse Education and Training has been approved for a maximum of 9.25 continuing education hours through the Oklahoma State Board of Licensed Social Workers, the Oklahoma Board of Examiners of Psychologists, the Licensed Marital and Family Therapist Committee, the Oklahoma Board of Licensed Alcohol and Drug Counselors and the Licensed Professional Counselors Committee.  Continuing education is also approved for the Certified Case Managers and Peer Recovery Support Specialists.  Continuing education for Licensed Behavioral Practitioners, Certified Prevention Specialists and the Council for Law Enforcement Education has been applied for and is pending approval.


Application for 1.0 hours of Category B Continuing Education credits has been approved to the Oklahoma Board of Dentistry for Dental Hygienists, Dentists, and other Oral Health professionals for the Oral Health Section meeting speaker.  Participants must sign in on the Oklahoma Board of Dentistry sign-in sheet to receive credit.

Sponsored by Oklahoma Public Health Association, a designated provider of continuing education contact hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc, this program is designated for Certified Health Education Specialists (CHES) and/or Master Certified Health Education Specialists (MCHES) to receive up to a total of 7.5 Category I contact education contact hours. Maximum advanced level continuing education contact hours available are 6.5.
